12th Grade Survey
Your school district is interested in your opinion about the types of information and activities that were provided while you were in your school district (in any grades kindergarten through 12th grade). It is important to answer the questions carefully, so that your district’s results are as complete and accurate as possible. The questions will take only a few minutes to complete.
For each question, decide whether you agree, tend to agree, tend to disagree or disagree. If you think the item is asking you about something that did not happen in your school district, mark ‘not applicable’ and skip to the next item.
Instructions: Click the 12th Grade Survey link below. You should have been issued a pin number from your school. There are 20 questions. When you are finished, click the Submit button at the bottom of the survey. Then, close your browser.
